Job Summary

Northamptonshire Healthcare Foundation Trust NHS is seeking a skilled Occupational Therapist to join our Community Rapid Response and Intermediate Care Team. As a key member of our team, you will play a vital role in supporting patients in their own homes, promoting independence, and facilitating timely discharges from hospital.

Main Responsibilities
  • Work collaboratively with multidisciplinary teams to assess and develop rehabilitation plans for patients.
  • Conduct home visits to assess patients' needs and provide expert advice on equipment and adaptations.
  • Develop and implement individualized care plans to promote independence and mobility.
  • Work closely with healthcare assistants to provide support and guidance.
  • Contribute to the development of the team's knowledge and skills through training and education.

The successful candidate will have a valid UK driving license and access to a vehicle for work purposes. You will be required to work a shift pattern that includes weekends and bank holidays.

Requirements
  • HCPC registration and relevant degree/qualification in occupational therapy.
  • 12 months NHS post-registration experience.
  • Ability to work as an autonomous practitioner.
  • Good communication and interpersonal skills.
  • Manual handling experience.
