About the Role

We are seeking an experienced Occupational Therapist to join our Community Rapid Response team at Northamptonshire Healthcare Foundation Trust. As an Occupational Therapist, you will play a vital role in supporting patients in their own homes, helping them to regain independence and improve their quality of life.

Key Responsibilities

  • Assess patients' needs and develop personalized care plans to promote independence and well-being
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support
  • Conduct home visits to assess patients' environments and provide recommendations for adaptations and equipment
  • Provide education and training to patients, carers, and healthcare professionals on occupational therapy techniques and strategies
  • Contribute to the development and implementation of new services and programs to improve patient outcomes

Requirements

  • HCPC registration as an Occupational Therapist
  • Experience working in a community setting, preferably in a rapid response or intermediate care team
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours and locations
