Job Summary

Northamptonshire Healthcare Foundation Trust NHS is seeking a skilled Occupational Therapist to join our Community Rapid Response and Intermediate Care team. As a key member of our multidisciplinary team, you will play a vital role in supporting patients in their own homes, promoting rehabilitation, and preventing hospital admissions.

Main Responsibilities
  • Work collaboratively with nurses, doctors, and support workers to develop and implement patient care plans.
  • Conduct assessments and provide expert advice on occupational therapy interventions to support patients in their daily lives.
  • Develop and maintain effective relationships with patients, families, and carers to ensure seamless care delivery.
  • Participate in multidisciplinary team meetings to discuss patient progress and develop care plans.
  • Contribute to the development and implementation of new services and initiatives to improve patient outcomes.
Requirements
  • HCPC registration and relevant degree/qualification in occupational therapy.
  • Minimum 12 months NHS post-registration experience.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
  • Manual handling experience and ability to use hoists and stand aids.
