We are seeking an experienced Occupational Therapist to join our Community Rapid Response and Intermediate Care Team in Northamptonshire. As a key member of our team, you will play a vital role in supporting patients in their own homes, promoting independence, and preventing hospital admissions.
Main Responsibilities- Work collaboratively with multidisciplinary teams to assess and develop rehabilitation plans for patients.
- Conduct home visits to assess patients' needs and provide expert advice on equipment and adaptations.
- Develop and implement individualized care plans to promote independence and well-being.
- Work closely with healthcare assistants to provide support and guidance.
- Contribute to the development of the team's knowledge and skills through training and education.

Requirements- HCPC registration and relevant degree/qualification in occupational therapy.
