About the Role


Northamptonshire Healthcare Foundation Trust is seeking an experienced Occupational Therapist to join our Community Rapid Response team. As an integral part of our multidisciplinary team, you will work closely with healthcare professionals to deliver high-quality care to patients in their own homes.


Key Responsibilities



  • Conduct comprehensive assessments to identify patients' needs and develop personalized care plans.
  • Collaborate with healthcare assistants and other team members to deliver effective care interventions.
  • Provide expert clinical guidance and support to colleagues to enhance their knowledge and skills.
  • Work closely with community teams to ensure seamless transitions from hospital to community care.

Requirements



  • HCPC registration and relevant degree/qualification in occupational therapy.
  • 12 months NHS post-registration experience.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Good communication and interpersonal skills.
