About the Role

We are seeking a skilled and experienced Senior Primary Care Nurse Practitioner to join our team at Derby Family Medical Centre. This is a challenging yet rewarding role that requires strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality patient care.

Key Responsibilities
  • To work autonomously as a senior member of our primary care team, providing comprehensive assessments, diagnoses, and treatments for patients with undifferentiated and undiagnosed illnesses or injuries.
  • To attend to patients who wish to be seen by a healthcare professional, conducting thorough physical assessments, and developing continuing management plans in collaboration with patients.
  • To prioritize and triage patient needs, making necessary referrals for investigations and ensuring timely delivery of care while meeting contractual requirements.
  • To develop and maintain strong working relationships with GPs, other healthcare professionals, and administrative staff to ensure seamless coordination of patient care.
Requirements and Qualifications
  • A minimum of 3 years of experience in primary care, with a proven track record of delivering high-quality patient care.
  • Prescriber qualification and a deep understanding of independent prescribing within the scope of professional practice.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and other stakeholders.
  • Demonstrated expertise in safe clinical decision-making, prioritization, and problem-solving, with the ability to adapt to changing situations and priorities.
  • Strong leadership and mentoring skills, with the ability to support and guide junior staff members in their development.

Salary and Benefits
