Dietitian Role in Primary Care

About the Role

Primary Care Dietitians is seeking a highly skilled and senior dietitian to join our team as a First Contact Dietitian (FCD) in Derby City North PCN. This is an in-person role that requires the dietitian to work collaboratively with the general practice multidisciplinary team (MDT) to meet the needs of patients.

Main Responsibilities

  • Assess, diagnose, triage, and manage patients with dietetic problems and undifferentiated conditions.
  • Develop and implement person-centred interventions for patients, including supported patient self-management and programmes to optimise health and nutrition.
  • Work as part of the MDT to create stronger links for dietetic services through clinical leadership, teaching, and evaluation skills.
  • Support research and audit programmes to evaluate and improve the effectiveness of our service.
  • Streamline the pathway of care by providing a responsive service for timely access to care.

Requirements

  • Fully HCPC and BDA registered practicing Dietitian.
  • Minimum 3 years of clinical experience in dietetics.
  • An up-to-date understanding of Health Education England's Roadmap to Practice.

Desirable Qualifications and Experience

  • Completion of some level 7 MSc learning.
  • Previous clinical experience of first contact triage clinical work.
  • Previous experience of using EMIS/SystmOne.
