About the Role

Hampshire County Council is seeking a highly skilled and experienced Social Worker to join our Learning Disabilities Team. As a key member of our team, you will be responsible for conducting comprehensive assessments, working collaboratively with individuals, families, and other professionals to achieve positive outcomes.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of individuals' needs, strengths, and risks to inform effective care plans.
  • Collaboration and Communication: Work effectively with individuals, families, and other professionals to achieve positive outcomes, ensuring seamless communication and coordination.
  • Safeguarding and Risk Management: Identify and mitigate risks, ensuring the safety and well-being of individuals and their families.
  • Case Management and Supervision: Manage a complex caseload, providing supervision and guidance to Case Workers and Senior Case Workers as needed.
  • Relationship Building: Develop and maintain positive, professional relationships with individuals, families, and colleagues.
Requirements
  • Qualifications and Experience: Qualified Social Worker, registered with Social Work England (SWE), with significant UK-based experience. We are unable to consider Newly Qualified Social Workers for this post.
  • Strengths-Based Approach: Demonstrate a sound understanding of how to apply a strengths-based approach in practice.
  • Knowledge and Skills: Possess a strong understanding of social work issues, legal frameworks, and social services' statutory duties.
  • Communication and Adaptability: Ability to adapt to individuals' communication needs and preferences.
  • Organisational Values: Commitment to demonstrating the organisation's values and behaviours.
