About the Role

We are seeking a highly motivated and compassionate Support Worker to join our team at Hope House in Slough. As a Support Worker, you will play a vital role in providing support and care to our customers, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Undertake key-working responsibilities for a caseload assigned by the Team Leader/Manager
  • Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
  • Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills
  • Develop and maintain links with all key agencies and service providers in the local community
  • Empower customers to ensure they receive the service and benefits they are entitled to
  • Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards
  • Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible
About You

We are looking for someone who is passionate about providing high-quality care and support to our customers. You will be a team player who is able to work collaboratively with colleagues and other agencies to achieve the best possible outcomes for our customers.

You will have a strong understanding of the needs of individuals with mental health issues and be able to provide support and guidance to help them achieve their goals.
