We are seeking an Independent Living Support Coordinator to join our team in Northamptonshire. As an Independent Living Support Coordinator, you will be responsible for coordinating the delivery of care services to customers in their own homes within a wider community.

Job Description

You will work closely with customers, families, and other healthcare professionals to ensure that customers receive the support and care they need to live independently.

  • Coordinate the delivery of care services, including arranging visits and ensuring that customers receive the support they need.
  • Monitor and evaluate the effectiveness of care services and make recommendations for improvement.
  • Work closely with other healthcare professionals to ensure that customers receive comprehensive care and support.
Requirements
  • Care Certificate (or willing to work towards).
  • Level 2 or above in Health and Social Care or equivalent (or willing to work towards).
  • Proven experience in health or care work, including coordination experience.
  • Experience of delivering care services in a community-based setting.

Salary: £18,000 - £22,000 per annum, depending on experience.
