Early Intervention Psychosis Care Coordinator

We are seeking a highly skilled and experienced Care Coordinator to join our Early Intervention in Psychosis Team in North Warwickshire and Rugby. The successful candidate will work with individuals who are experiencing the first onset of psychosis, providing comprehensive care and support to help them achieve their goals and improve their overall well-being.
Main ResponsibilitiesThe Care Coordinator will be responsible for conducting detailed initial assessments, incorporating the CAARMs assessment to identify at-risk mental states or first-episode psychosis. They will work collaboratively with the client to develop a personalized care plan, incorporating a recovery philosophy to support the individual's goals and needs. The Care Coordinator will also be responsible for managing risk, utilizing the Steve Morgan tools, and working creatively with other team members and independently to support the client's progress.
About the RoleThis is a challenging yet rewarding role that requires a high level of expertise, compassion, and enthusiasm. The successful candidate will have significant experience in community care coordination, a strong understanding of holistic health and well-being, and excellent IT skills. They will also be required to have a full driving license and access to a motor vehicle for business use.
Person SpecificationThe ideal candidate will possess a relevant professional qualification, such as an RMN or DipCOT, and current unrestricted registration with the HCPC/NMC. They will also have substantial experience in a community mental health care setting, with a strong understanding of psychosis and risk assessment. The successful candidate will be able to demonstrate a commitment to ongoing professional development and a passion for improving the lives and well-being of individuals with mental health needs.
