Job Summary

The LD and Autism Transforming Care Specialist will play a crucial role in the Transforming Care Programme, working to improve health outcomes for individuals with learning disabilities and/or autism.

Key Responsibilities

  • Provide specialist advice and professional expertise to health professionals and social workers
  • Demonstrate expert and effective practice in complex situations
  • Work with colleagues to mitigate high levels of risk and make independent decisions
  • Coordinate community services to enable individuals to remain living in the community
