As a key member of our Transforming Care Programme, you will play a vital role in transforming health and social care systems to deliver improved health outcomes for people with learning disabilities and/or autism.
Job Summary:The Learning Disability and Autism Case Manager will be responsible for enabling preventative and integrated approaches to support autistic adults and/or those with learning disabilities who display behaviours that challenge. This will involve providing specialist advice and professional expertise to health professionals and social workers, demonstrating expert and effective practice in complex situations, and working with and supporting colleagues to mitigate high levels of risk.
Key Responsibilities:- Provide specialist advice and professional expertise to health professionals and social workers

- Independent decision making
