Main Responsibilities:

  • Collaborate with Occupational Therapists to assess the needs of adults in their homes and community settings, ensuring compliance with relevant legislation.
  • Conduct thorough assessments of service users and carers with complex needs, focusing on home environment adaptations.
  • Support the development of personalized support plans in collaboration with multi-disciplinary teams.
  • Facilitate reablement services for hospital discharge, emphasizing goal setting, regular review, and interventions to maximize recovery and independence.
  • Provide expert advice and assessments for equipment and home modifications, prioritizing client-centered principles.
  • Recommend adaptations and signpost resources to support service users.
  • Assist Occupational Therapists with moving and handling assessments, ensuring the safety and dignity of service users.
  • Maintain awareness of safeguarding principles and the importance of respecting individuals' dignity.

Requirements:

  • Previous experience working with Occupational Therapists in community settings.
  • Background in adaptations and manual handling.
  • Enhanced DBS/CRB clearance (arranged by the employer).
  • Eligibility to work in the UK.
