About the Role

We are seeking a skilled and dedicated Community Nurse to join our team in Hertfordshire. As a Community Nurse, you will be responsible for providing high-quality care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care delivery.

The ideal candidate will have recent clinical nursing experience, with a strong background in community nursing and a passion for delivering holistic care. You will be confident in your ability to assess, plan, and evaluate care for a range of conditions, and have excellent communication and interpersonal skills.

Key Responsibilities
  • Provide high-quality care to patients in their own homes, working closely with other healthcare professionals.
  • Assess, plan, and evaluate care for a range of conditions, using evidence-based practice.
  • Develop and maintain strong relationships with patients, families, and carers, ensuring person-centred care.
  • Work collaboratively with the multidisciplinary team to ensure seamless care delivery.
  • Participate in ongoing education and training to maintain and develop your skills and knowledge.
Requirements
  • Registered Nurse with current UK NMC registration.
  • Recent clinical nursing experience in a community setting.
  • Strong background in community nursing and a passion for delivering holistic care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
