About the Role

We are seeking a highly skilled and experienced Paediatric Sickle Cell Nurse Practitioner to join our team at Hertfordshire Community NHS Trust. The successful candidate will be responsible for providing expert care and support to children and young people with Sickle Cell Disease, as well as their families.

Key Responsibilities

  • Provide expert assessment, planning, and evaluation of care for children and young people with Sickle Cell Disease
  • Develop and implement individualised care plans to meet the needs of each patient
  • Work closely with other healthcare professionals to ensure seamless care and support
  • Provide education and training to patients, families, and healthcare professionals on Sickle Cell Disease
  • Contribute to the development of policies and procedures related to Sickle Cell Disease care

Requirements

  • Registered Children's Nurse with a Specialist Practitioner qualification
  • Experience of caring for children and young people with Sickle Cell Disease in a community or acute setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Sound knowledge and understanding of Safeguarding processes
