Job Summary

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team as a Medicines Optimisation Specialist. The successful candidate will play a key role in leading our medicines optimisation efforts, working closely with our primary care team to improve patient safety and quality of care.

Main Responsibilities
  • Lead the development and implementation of medicines optimisation strategies to improve patient outcomes and reduce healthcare costs.
  • Work closely with our primary care team to identify areas for improvement and develop targeted interventions to address these issues.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Develop and maintain relationships with key stakeholders, including patients, families, and healthcare providers.
  • Stay up-to-date with the latest evidence-based practices and guidelines to inform medicines optimisation decisions.
Requirements
  • Minimum of 2 years post-qualification experience in a primary care setting.
  • Good knowledge of NHS medicines optimisation principles and practices.
  • Experience of managing and developing a team.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work outside of core office hours.
