Mental Health Clinician - Older Adults
Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust is seeking a skilled and experienced clinician to join our Older Adult Community Treatment Team. As a Band 6 Community Practitioner, you will coordinate mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions, focusing on care homes.

Responsibilities
• Coordinate mental health care and treatment for patients with cognitive and functionally frail conditions
• Work collaboratively with healthcare professionals to deliver high-quality patient care
• Develop and implement individualized care plans tailored to each patient's needs
• Provide clinical and psychologically based interventions to promote recovery and well-being
• Collaborate with service users, carers, and healthcare professionals to ensure continuity of care

Requirements
• Professional qualification and current professional registration as a First-Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist
• Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post
• Experience working with older adults with complex mental health needs
• Excellent communication and interpersonal skills
• Ability to work collaboratively as part of a multidisciplinary team
