About the Role

We are seeking a caring and empathetic Care Coordinator to join our growing team and be the first point of contact for our patients. The role is a mixture of patient-facing, answering telephone calls, greeting patients face-to-face, and administrative duties such as processing referrals, registrations.

Key Responsibilities
  • Receive, assist, and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective way.
  • Offer general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.
  • Undertake a variety of administrative duties to assist in the smooth running of the practice, including the provision of secretarial and clerical support to clinical staff and other members of the practice team.
  • Facilitate effective communication between patients, members of the primary healthcare team, secondary care, and other associated healthcare agencies.

Our main surgery is based in a purpose-built Centre, with a growing list size of 15,000+ and are rated Outstanding by the CQC. We hold clinics from Brixham Hospital and a branch surgery in Galmpton village.

We are part of the Baywide PCN, a dynamic, forward-thinking network of three Torbay practices with a patient population of 50,000 spread across Brixham, Paignton, and Torquay.
