About the Role

We are seeking a highly motivated and experienced Social Prescribing Link Worker to join our team at Primary Healthcare Darlington. As a key member of our team, you will play a vital role in empowering individuals to take control of their health and wellbeing through referral to a social prescribing link worker.

Key Responsibilities
  1. Take referrals from a wide range of agencies, including GP practices, pharmacies, and community organizations, to provide personalized support to individuals and their families.
  2. Develop trusting relationships with individuals, their families, and carers, providing non-judgmental support and respecting diversity and lifestyle choices.
  3. Co-produce a personalized wellbeing plan with individuals, focusing on their priorities, interests, values, and motivations, and connecting them to community groups and activities.
  4. Work with local agencies to raise awareness of social prescribing and provide regular updates on the development of the service.
  5. Manage expectations and provide clear information about the groups, activities, and services individuals are being connected to.
Requirements
  • GCSE/NVQ level 2/3 or equivalent level of training and experience.
  • Understanding of basic health and social care terminology.
  • Proven ability to recognize and manage risk.
  • Good interpersonal skills and ability to communicate confidently with staff at all levels.
  • Ability to work well across teams and prioritize tasks effectively.
