Social Prescribing Link Worker

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Primary Healthcare Darlington. As a key member of our Wellbeing Service, you will play a vital role in empowering individuals to take control of their health and wellbeing.
Key Responsibilities- Develop and maintain strong relationships with local agencies, GP practices, and community groups to promote the Wellbeing Service and facilitate referrals.
- Conduct comprehensive wellbeing assessments, identifying the wider determinants of health and developing personalized plans to improve health outcomes.
- Provide non-judgmental support, building trust with individuals, their families, and carers, and connecting them to community groups and statutory services.
- Manage and prioritize caseloads, ensuring timely and effective support, and referring individuals to other health professionals or agencies when necessary.
- Collaborate with local partners to develop and sustain community groups, services, and initiatives, promoting community resilience and social prescribing.
- Collect and analyze data to track the impact of social prescribing on health and wellbeing, informing service development and improvement.
- GCSE/NVQ level 2/3 or equivalent level of training and experience.
- Understanding of basic health and social care terminology.
- Proven ability to recognize and manage risk, knowledge of the needs of older people, safeguarding, and the associated legislative framework.
- Good interpersonal skills, ability to communicate confidently with staff at all levels.
- Ability to work well across teams, manage time effectively, and prioritize tasks.
- IT skills and experience in the use of Microsoft Office programs.
- Familiarity with IT systems currently in use in health and social care settings.
- Experience in working directly in a community development context, adult health and social care, learning support, or public health/health improvement.
- Experience in supporting people, their families, and carers in a related role.
- Experience in supporting people with their mental health, either in a paid, unpaid, or informal capacity.
- Experience in working with the VCSE sector, including with volunteers and small community groups.
