About the Role

We are seeking a highly motivated and experienced Social Prescribing Link Worker to join our team at Primary Healthcare Darlington. As a key member of our team, you will play a vital role in empowering individuals to take control of their health and wellbeing through referral to our social prescribing service.

Key Responsibilities
  1. Take referrals from a wide range of agencies, including GP practices, pharmacies, and community organizations, to provide personalized support to individuals and their families.
  2. Develop trusting relationships with individuals, their families, and carers, providing non-judgmental support and respecting diversity and lifestyle choices.
  3. Co-produce personalized wellbeing plans with individuals, connecting them to community groups and activities that promote health and wellbeing.
  4. Work collaboratively with local agencies, including GP practices, to raise awareness of social prescribing and encourage referrals.
  5. Provide regular updates to referral agencies about the development of the service and offer training on how to access information to encourage appropriate referrals.
Requirements
  • GCSE/NVQ level 2/3 or equivalent level of training and experience.
  • Understanding of basic health and social care terminology.
  • Proven ability to recognize and manage risk.
  • Good interpersonal skills and ability to communicate confidently with staff at all levels.
  • Ability to work well across teams and prioritize tasks effectively.
