Job Summary

The Living Well Service is seeking a skilled Social Prescribing Link Worker to empower individuals to take control of their health and wellbeing. As a key member of our team, you will work collaboratively with local agencies to provide personalized support and connect individuals with community groups and statutory services.

Main Responsibilities
  1. Develop trusting relationships with individuals, their families, and carers to understand their unique needs and priorities.
  2. Co-produce personalized wellbeing plans to improve health and wellbeing, focusing on community connections and statutory services.
  3. Work with local agencies to raise awareness of social prescribing and encourage referrals.
  4. Provide regular updates and training to referral agencies on the development of the service.
  5. Encourage self-referrals and connect with local communities, particularly those hard to reach.

Key Requirements
  • GCSE/NVQ level 2/3 or equivalent level of training and experience.
  • Understanding of basic health and social care terminology.
  • Proven ability to recognize and manage risk.
  • Good interpersonal skills and ability to communicate confidently with staff at all levels.
  • Ability to work well across teams and manage priorities effectively.
Desirable Qualifications and Experience
  • Familiarity with IT systems currently in use in health and social care settings.
  • Experience in working directly in a community development context, adult health and social care, learning support, or public health/health improvement.
  • Experience in supporting people, their families, and carers in a related role.
