About the Role

Primary Healthcare Darlington is seeking a skilled Link Worker to join our team in delivering high-quality social prescribing services to the community. As a Link Worker, you will play a vital role in empowering individuals to take control of their health and wellbeing by connecting them with community groups and statutory services.

Key Responsibilities

  • Take referrals from various agencies, including GP practices, pharmacies, and social care services, to provide personalized support to individuals and their families.
  • Develop trusting relationships with clients, giving them time to focus on their priorities and needs.
  • Co-produce personalized wellbeing plans with clients, connecting them to community groups and activities that promote their health and wellbeing.
  • Work collaboratively with local agencies to raise awareness of social prescribing and encourage referrals.
  • Provide regular updates to referral agencies about the development of the service and offer training on how to access information to encourage appropriate referrals.

Requirements

  • GCSE/NVQ level 2/3 or equivalent level of training and experience.
  • Understanding of basic health and social care terminology.
  • Proven ability to recognize and manage risk, knowledge of the needs of older people, safeguarding, and the associated legislative framework.
  • Good interpersonal skills, ability to communicate confidently with staff at all levels, and work well across teams.
  • Ability to work on own initiative without direct supervision, understanding where clinical input is needed.
  • IT skills and experience in the use of Microsoft Office programmes.
