Social Prescribing Link Worker

We are seeking a highly motivated and experienced Social Prescribing Link Worker to join our team at Primary Healthcare Darlington. As a key member of our team, you will play a vital role in empowering individuals to take control of their health and wellbeing through referral to our social prescribing service.
Key Responsibilities- Take referrals from a wide range of agencies, including GP practices, pharmacies, and community organizations, to provide personalized support to individuals and their families.
- Develop trusting relationships with clients, providing non-judgmental support and respecting their diversity and lifestyle choices.
- Co-produce personalized wellbeing plans with clients, connecting them to community groups and activities that promote their health and wellbeing.
- Work collaboratively with local agencies to raise awareness of social prescribing and encourage referrals.
- Provide regular updates to referral agencies about the development of the service and offer training on how to access information to encourage appropriate referrals.
- GCSE/NVQ level 2/3 or equivalent level of training and experience.
- Understanding of basic health and social care terminology.
- Proven ability to recognize and manage risk, knowledge of the needs of older people, safeguarding, and the associated legislative framework.
- Good interpersonal skills, ability to communicate confidently with staff at all levels, and work well across teams.
- Ability to work on own initiative without direct supervision, understanding where clinical input is needed.
- IT skills and experience in the use of Microsoft Office programmes.
