Social Work Professional

This role offers an exciting opportunity to work as a Case Worker in one of our Learning Disabilities Community teams, providing support and guidance to individuals with the goal of maximizing their wellbeing.
About the RoleAs a Social Work Professional, you will utilize a strengths-based approach to assess individual needs and provide tailored support. This involves collaborating with individuals, their families, providers, and other healthcare professionals to develop effective strategies for addressing life's challenges and promoting independence and safety within the community.
Key Responsibilities- Develop and implement personalized support plans that focus on individuals' resourcefulness, strengths, and abilities.
- Maintain effective relationships with individuals, their families, and relevant stakeholders through active listening, empathy, and open communication.
- Collaborate with experienced colleagues and qualified social/healthcare professionals to ensure comprehensive support and guidance.
To be successful in this role, you will need to possess excellent relationship management and negotiation skills, with the ability to adapt to diverse situations and individuals. A strong understanding of strengths-based practice principles is essential, as well as the capacity to work independently and as part of a team.
Salary and BenefitsThe estimated salary range for this position is £26,918 - £29,616 per annum (pro-rata), which translates to approximately £17,096.57 - £18,810.16 per annum based on 23.5 hours per week. In addition to competitive remuneration, we offer a range of benefits, including regular supervision, mentoring, and Continuous Professional Development opportunities, as well as a supportive working environment.
Travel RequirementsYou will be required to travel to various locations within Hampshire, necessitating a valid driver's license and access to a reliable vehicle on a regular basis.
