Job Title: Health and Social Care Coordinator

Margaret Clitherow Trust is seeking a dedicated and passionate individual to join our team as a Health and Social Care Coordinator.

About the Role:
As a Health and Social Care Coordinator, you will work closely with individuals and families who have complex health and social care needs, advocating for them and empowering them to navigate healthcare and social care systems.

Key Responsibilities:
- Develop strong relationships with health and social care providers to tackle health inequalities and achieve the best outcomes for our communities and clients.
- Work with vulnerable people to support them in accessing the care they need.

Requirements:
- Experience of successful work with vulnerable people and an understanding of health and social care systems.
- Ability to work in a hybrid environment with some home working.
- Valid UK driving license and use of own car.
