Learning Disability Social Worker

We are looking for an experienced social worker to join our team in providing high-quality care and support to adults with learning disabilities.
This is a permanent, full-time role with hybrid working, offering a salary of £37,080 to £40,788 per annum.
As a key member of our multi-disciplinary community team, you will:
- Work closely with individuals, families, and carers to improve their lives.
- Conduct assessments, undertake care reviews, and support care planning.
- Take part in a duty rota to address incidents, safeguarding alerts, and information requests.
- Undertake safeguarding investigations, capacity assessments, and best interest decisions.
Additionally, you will promote self-care and management of conditions, improve integrated working, and engage with the third sector.
