
Care Coordinator for Adults with Learning Disabilities

This is a full-time, permanent role in the Royal Borough of Windsor and Maidenhead's Community Team for People with Learning Disabilities. The successful candidate will have experience working with adults with learning disabilities and their families and carers.
About the RoleYou will be working as a social worker to help adults with learning disabilities lead as full and fulfilling lives as possible. This includes carrying out assessments, undertaking care reviews and supporting care planning.
The role involves working within a multi-disciplinary community team, supporting adults with learning disabilities and their families and carers to improve their lives.
Key Responsibilities- Carry out assessments and undertake care reviews to support adults with learning disabilities
- Support care planning to enable adults with learning disabilities to live fulfilling lives
- Take part in a duty rota to deal with incidents, safeguarding alerts and information requests
- Undertake safeguarding investigations and capacity assessments
Salary: £37,080 to £40,788 per annum
RequirementsWe are looking for an experienced social worker who has a good understanding of safeguarding protocols and has worked in a health and social care environment.
