About the Role

We are seeking a highly skilled and dedicated Care Coordinator to join our Perinatal Mental Health Team. As a key member of our multi-disciplinary team, you will work closely with patients, their families, and carers to provide comprehensive care and support.

Key Responsibilities
  • Complete assessments and develop care plans in collaboration with patients and their families
  • Provide support and guidance to patients and their families, promoting independence and well-being
  • Work closely with other healthcare professionals to ensure seamless care and support
  • Participate in regular team meetings and contribute to the development of care pathways

Requirements
  • Appropriate degree-level qualification in Mental Health Nursing, Occupational Therapy, or Social Work
  • Experience of working in a community mental health setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
