We are seeking a highly skilled and dedicated Care Coordinator to join our Perinatal Mental Health Team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in supporting women at risk of developing or experiencing moderate to severe mental health difficulties during pregnancy and the postnatal period.
Key Responsibilities- Work closely with our team to provide comprehensive assessments and support to patients and their families
- Develop and implement individualized care plans to promote mental health and well-being
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication
- Provide education and support to patients and their families on mental health issues and coping strategies
- Participate in regular team meetings and contribute to the development of new initiatives and programs
- Appropriate degree-level qualification in Mental Health Nursing, Occupational Therapy, or Social Work
- Experience working in a community mental health setting, preferably in a perinatal or maternal mental health role
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
- Ability to work independently and as part of a team, with a flexible and adaptable approach
- Knowledge of mental health legislation, policies, and procedures, including the Mental Health Act 1983 and the Care Programme Approach
