About the Role

We are seeking a highly skilled and dedicated Care Coordinator to join our Perinatal Mental Health Team. As a key member of our multi-disciplinary team, you will work closely with patients, their families, and carers to provide comprehensive care and support.

Key Responsibilities
  • Complete assessments and develop care plans in collaboration with patients and their families
  • Support patients in group settings and provide individualized care as needed
  • Work closely with other members of the MDT to ensure seamless care coordination
  • Supervise and teach junior staff to ensure continuity of care
