About the Role

We are seeking a skilled and compassionate Mental Health Nurse to join our team in providing high-quality care to individuals experiencing homelessness and mental health issues. As a key member of our Community Mental Health Team, you will work closely with clients, colleagues, and external agencies to deliver a comprehensive and coordinated service.

Key Responsibilities
  • Contact and engage with rough sleepers, homeless individuals, and those in day centers and hostels to provide mental health support and interventions.
  • Conduct comprehensive assessments of mental health, housing, and social care needs to develop personalized care plans.
  • Offer short-term interventions to link clients into mainstream services and provide ongoing care management responsibilities.
  • Collaborate with the Operational Team Manager, Clinical Leads, and other stakeholders to ensure a seamless service delivery.
  • Participate in team meetings, support groups, and other routine meetings to contribute to the development of policies and procedures.
  • Undertake regular street outreach, working assertively with clients who may be resistant to contact, and liaise with other outreach teams to ensure a comprehensive service.
Requirements
  • RMN with current NMC registration.
  • Experience working in a setting that provided care for people with acute and chronic mental health issues.
  • Understanding of government policy on mental health, homelessness, and rough sleeping.
  • Effective written and verbal communication skills.
  • Ability to work independently and as part of a team.
