Key Responsibilities
  • To assess patient needs and develop comprehensive care plans, taking into account their medical history, social circumstances, and mental health status.
  • To provide evidence-based interventions, including cognitive-behavioral therapy, medication management, and crisis intervention, to patients with complex mental health needs.
  • To work collaboratively with other healthcare professionals, including doctors, nurses, and social workers, to ensure coordinated care delivery and achieve positive patient outcomes.
  • To participate in multidisciplinary team meetings to discuss patient progress, share knowledge, and plan care strategies.
  • To maintain accurate and up-to-date records of patient interactions, assessments, and treatment plans, adhering to relevant policies and guidelines.
Requirements
  • Mental Health Practitioner registration (Registered Mental Health Nurse Social Worker or Occupational Therapist).
  • Evidence of ongoing professional development since registration.
  • Strong understanding of mental health assessment, diagnosis, and treatment principles.
  • Ability to work effectively with diverse populations, including those from minority ethnic backgrounds.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare teams.

We offer a competitive salary of £44,400 - £62,000 per annum, depending on experience, plus a range of benefits, including access to training and development opportunities, flexible working arrangements, and a supportive work environment.
