Job Title: Clinical Nursing Practitioner

We are seeking a dedicated and forward-thinking nurse to join our team as a Clinical Nursing Practitioner.

About the Role:

This is an exciting opportunity for a registered general nurse with a passion for delivering excellent patient care. The successful candidate will have experience in primary care, preferably with supporting qualifications in Long Term Condition management and treatment room procedures.

The ideal candidate will be able to work collaboratively in a team, provide compassionate and inclusive care for patients from all backgrounds, and maintain accurate records.

Main Responsibilities:
  • Manage patients newly diagnosed and ongoing healthcare using practice template protocols/NICE Guidance and local guidelines.
  • Undertake annual review of patients understanding and ability to self-manage.
  • Refer to other healthcare professionals as necessary
  • Maintain a disease register.
  • Audit outcomes of care against national and local standards and initiate change as necessary.
  • Offer treatment room activities such as vaccinations and immunisations and cytology.
Requirements:
  • Registered General Nurse qualification with current NMC registration
  • Experience in a primary care setting (desirable but not essential) with supporting qualifications in Long Term Condition management and treatment room procedures.
  • Excellent communication skills and the ability to work collaboratively in a team
  • A commitment to providing compassionate and inclusive care for patients from all backgrounds
Salary:

The salary for this role is £35,000 - £40,000 per annum, depending on experience.
