About the Role

We are seeking a highly motivated and enthusiastic nurse to join our dynamic team as an NHS Specialist Health Nurse. This is an ideal opportunity for the successful applicant to further develop their skills and contribute to the delivery of high-quality patient care.

Main Responsibilities

The successful candidate will be responsible for providing a high-quality service that is patient-centered, working closely with the multidisciplinary team to deliver care that meets the needs of our patients. Key responsibilities will include:

  • Providing expert nursing care to patients on anticoagulation treatments
  • Developing and implementing individualized care plans to meet the needs of patients
  • Collaborating with other healthcare professionals to ensure seamless care delivery
  • Participating in teaching and learning initiatives to enhance the skills of colleagues
  • Conducting audits and quality initiatives to improve patient outcomes
Person Specification

We are looking for a nurse with significant post-registration experience, preferably in a specialist role or with experience caring for patients on anticoagulation treatments. The successful candidate will be a non-medical prescriber or be willing to work towards this qualification. They will also have a full UK driving license and access to service fleet vehicles.
