Nutrition Nurse Specialist

We are seeking a highly skilled and dedicated Nutrition Nurse to join our team at Sandwell and West Birmingham NHS Trust. As a Nutrition Nurse, you will play a vital role in providing high-quality patient care and support to individuals requiring nutritional support via enteral feeding tubes.
Key Responsibilities- Assess and provide individualized nursing care to patients with complex nutritional needs, working closely with multidisciplinary teams to ensure seamless care.
- Collaborate with healthcare professionals to develop and implement personalized nutrition plans, promoting optimal patient outcomes.
- Provide education and training to patients, carers, and healthcare staff on best practices for enteral feeding and nutrition support.
- Liaise with acute unit Nutrition Nurses, Hospital and Community Dietitians, GPs, and other stakeholders to facilitate smooth patient transitions and care coordination.
- Work proactively to prevent hospital admissions by responding promptly to patient and carer needs, ensuring timely access to nutritional support.
- Contribute to the development and implementation of nutrition-related policies and procedures, promoting evidence-based practice and quality improvement.
