Job Summary

We are seeking a Public Health Support Specialist to join our team in the All Wales Diabetes Prevention Programme. The successful candidate will work with patients to reduce their risk of type 2 diabetes by providing motivational interviewing and brief intervention training. The role involves working closely with General Practises and community settings to deliver targeted interventions and support individuals in making positive lifestyle changes.

The ideal candidate will have a Level 3 Diploma in Clinical Healthcare Support or equivalent qualification and experience in using information technology such as Microsoft office/outlook. A good understanding of local services and communities is also essential. The estimated salary for this role is £28,000 - £34,500 per annum depending on location and experience.

Main Duties

Advise on healthy living options and behaviours for individuals, in their context

Help individuals to develop an 'action plan' to change behaviour and sustain those behaviours, leading to more healthy choices and actions

Support individual's 'action plan' over an agreed time through practical help

Help individuals to identify and access local services

Keep agreed records and monitor progress

To accurately record clinical data
