About the Role

As a Healthcare Assistant, you will be working with our team to support individuals in improving their health and wellbeing. The role is part of the All Wales Diabetes Prevention Programme (AWDPP) aimed at reducing the risk of type 2 diabetes among high-risk groups.

The ideal candidate will have experience in healthcare settings and possess excellent communication skills to engage with patients and motivate them to make positive lifestyle changes. You will work closely with General Practises and community settings to deliver brief interventions and provide support to individuals at risk of developing type 2 diabetes.

Main Responsibilities

Advise on healthy living options and behaviours for individuals, in their context

Help individuals to develop an 'action plan' to change behaviour and sustain those behaviours, leading to more healthy choices and actions

Support individual's 'action plan' over an agreed time through practical help

Help individuals to identify and access local services

Keep agreed records and monitor progress

To accurately record clinical data

This post is fixed term for 18 months due to meet the needs of the service. Estimated salary: £28,000 - £34,500 per annum depending on location and experience.
