About the Role

As a Lead Support Worker with Home Group, you will be working closely with our customers who have complex dual diagnosis with severe and enduring mental health needs. Your role will involve creating support plans and coordinating a team of Support Workers to help our customers achieve their hopes and aspirations.

A Typical Day

">
  • Creating personalized support plans and coordinating with colleagues to ensure effective implementation.
  • Providing daily living support, such as assistance with meals, shopping, budgeting, and maintaining tenancy.
  • Conducting risk assessments, support planning, goal setting, and regular reviews.
  • Cultivating meaningful relationships with customers through regular check-ins and activities.

  • Passion for supporting customers with complex mental health needs.
  • Experience in creating person-centered support plans and collaborating with colleagues.

Job Details

">
  • We work a 3-week rolling rota, including days, sleep-in nights, weekends, and on-call shifts.
  • We offer flexible scheduling while meeting customer needs.
  • Able to use technology for support plan creation, online learning, and collaboration with colleagues.
