About the Role:

You will be part of a dynamic team that empowers individuals to live their best lives. As our Lead Support Worker, you'll support customers with complex dual diagnosis and severe mental health needs to develop skills for independent living in the community.

 

A Day in the Life:

  • Develop and implement personalized support plans to help customers achieve their goals.
  • Collaborate with colleagues to provide comprehensive care and empower customers to make informed decisions.
  • Conduct regular risk assessments, create support planning documents, and facilitate goal setting and reviews.

 

You Will Bring:

  • Possess a genuine passion for supporting customers to live fulfilling lives.
  • Experience in creating person-centered support plans and supporting colleagues.

 

Job Details:

  • We operate on a 3-week rolling rota, including days, sleep-in nights, weekends, and On Calls.
  • We require flexibility to meet customer needs.
  • You will receive regular training and team meetings during the daytime.

 

Salary: £24,664 per annum or £37.61 per hour
