We are seeking a highly skilled and experienced Band 6 Senior Community Nurse to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a Senior Community Nurse, you will play a key role in delivering high-quality care to adults with learning disabilities in our community.
About the Role:- Manage a caseload of clients with complex learning disabilities, providing assessment, identification, and treatment of their health needs.
- Contribute to the planning, development, and evaluation of learning disability services, including contributing to defined projects as delegated by the LDP management team.
- Work autonomously to manage a specialist caseload, providing assessment, identification, and treatment of the health needs of adults with a learning disability.
- Be highly proficient in the assessment, identification, treatment, and management of the specialist health and nursing needs of the learning disability population.
- RNLD registration with the Nursing and Midwifery Council.
- Registered Practice Educator or mentor in line with Nursing and Midwifery Council requirements.
- Experience of working with adults with learning disabilities.
- Experience of nursing assessment and intervention relevant to adults with learning disabilities.
- Ability to reflect and critically appraise own performance and of those you supervise.
- Mental Health Act and Mental Capacity Act knowledge.
- Awareness of current trends, legislation, and guidance in support for people with learning disabilities and their carers.
- Evidence of continuous professional development.
