Senior Community Health Care Professional

We are seeking a highly skilled nurse to join our team as a Lead Nurse for Adults with Learning Disabilities. As a Senior Community Health Care Professional, you will play a vital role in delivering person-centered care to adults with learning disabilities.
The successful candidate will have experience in a similar role or a desire to progress from Band 5 to Band 6. You will be responsible for managing a complex caseload, conducting comprehensive nursing assessments, creating care plans, and implementing nursing interventions.
You will work within a Multi-Disciplinary Team (MDT) alongside other health professionals to support individuals with learning disabilities in North and West Oxfordshire. This is an excellent opportunity to make a real difference in the lives of individuals with learning disabilities.
Salary and BenefitsWe offer a competitive salary of £32,933 - £43,041 per annum, depending on experience. In addition, you will receive a generous benefits package, including 27 days annual leave, plus bank holidays, rising to 33 days with continuous service.
