Community Care Professional

We are seeking a Recovery and Wellbeing Specialist to join our team, who will provide community-based support to patients and their carers, promoting positive outcomes and networking with other agencies. The successful candidate will have experience of working in a care setting and/or education/social care setting, with some knowledge of mental health. You will be self-reliant, able to work flexibly both individually and in a team, and have excellent communication skills.
The salary for this role is £28,000 - £32,000 per annum, depending on experience, and we offer excellent benefits including 27 days annual leave, plus bank holidays, rising to 33 days with continuous service. Key Responsibilities:
- To implement specific areas of the care plan that have been planned and designed by senior clinicians in the service.
- To promote a therapeutic relationship with agreed boundaries and time scales.
- Assist clients in the completion of standardised assessments in line with the service's assessment protocol.
