Care Coordinator

Cygnet Health Care is a leading provider of specialist mental health services. We are seeking a highly skilled and dedicated Social Worker to join our team at Cygnet Hospital Bierley.
About the RoleThis is a full-time position, working 40 hours per week, with opportunities for part-time arrangements. As a Social Worker, you will work closely with service users, their families, and other stakeholders to provide high-quality care and support.
Key Responsibilities:
- Provide social care support to service users in accordance with their individual needs and preferences.
- Collaborate with multidisciplinary teams to ensure comprehensive care planning and delivery.
- Develop and maintain relationships with community partners and external agencies to facilitate effective service user support.
- Ensure accurate and timely documentation of service user information and progress.
Requirements:
- A registered Social Worker with Social Work England.
- Relevant Social Work qualification.
- Experience in working within a mental health setting or a strong desire to do so.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
