About the Role

This is an excellent opportunity to work as a Complex Case Manager with Connect Executive Search Group, where you will have the chance to make a real difference in the lives of our clients.

Main Responsibilities:
  1. As a Complex Case Manager, you will be responsible for managing a caseload of clients with complex needs, working closely with their appointed Nurse Case Manager to provide holistic care.
  2. You will be expected to develop, review, and implement client care plans and risk assessments, ensuring that they meet the required standards.
  3. Excellent communication and interpersonal skills are essential for this role, as you will be working with clients, professionals, and other stakeholders to deliver high-quality care.
  4. You will also be responsible for maintaining accurate records and reporting any issues that may impact the quality of care or the health and safety of clients and healthcare workers.
