About the Role

We are seeking a skilled Complex Care Specialist to join our team at Cornerstone Community Care. As a key member of our staff, you will be responsible for providing high-quality support to individuals with complex care needs.

Your primary objective will be to empower these individuals to live independently and maintain their well-being. You will work closely with a multi-disciplinary team, including families and guardians, to develop and implement personalized support plans.

The ideal candidate will possess previous experience in providing care and support to adults with learning disabilities or forensic services. They should also have excellent communication skills, numeracy skills, and the ability to deal with challenging behaviors.

Salary: £25,000 - £30,000 per annum depending on experience
