Personalised Care and Support Coordinator

As a Complex Care Support Specialist at Cornerstone, you will play a vital role in supporting adults with complex needs to live fulfilling lives. Our service users require personalized care and support to manage daily living tasks, maintain independence, and engage in meaningful activities. As a member of our team, you will work collaboratively with service users, their families, and other stakeholders to develop and implement support plans that promote dignity, respect, and well-being.
Key Skills and Qualifications
- Previous experience in care and support provision, particularly with adults with learning disabilities
- Excellent communication skills, both verbal and written
- Numeracy skills to ensure accurate record-keeping
- Confidence in dealing with challenging behaviors
- Passion for delivering person-centered care and a positive approach to learning
What to Expect
You can expect a supportive and collaborative work environment, comprehensive training, and opportunities for career progression. A competitive salary of £25,000 - £30,000 per annum, depending on experience, is also offered.
