Job Title: Clinical Care Navigator Lead

Salary: £50,000 - £65,000 per annum

We are seeking a skilled and experienced Clinical Care Navigator Lead to join our team at Black Country Healthcare NHS Foundation Trust. In this role, you will be responsible for supporting clinical teams in optimizing patient care pathways, ensuring timely and effective triage and assessment.

About the Role:

  • Utilize clinical expertise to support safe patient referral pathways, ensuring appropriate care plans are developed.
  • Actively manage referral management capacity, ensuring timely triage and assessment.
  • Lead in managing patient flow, coordinating their journey through each referral pathway.

Requirements:

  • Degree in Nursing or relevant healthcare field.
  • Experience in mental health service management.
  • Strong clinical skills and knowledge.
