Main Responsibilities:
  • Provide clinical, hands-on care to patients in their homes, aiming to prevent unnecessary hospital admissions.
  • Offer specialist knowledge, expertise, and support to patients and carers, enabling them to stay in their homes if that is their wish.
  • Develop and maintain effective communication skills, with advanced training or willingness to undertake this training.
  • Work collaboratively with health and social care colleagues from other organizations to provide high-quality palliative and end of life care.
  • Travel using your own vehicle, holding a UK driving licence and relevant insurance.

Job Requirements:
  • Clinical Nurse Specialist/Paramedic Community planned care team

  • Salary: £35,388 - £44,117 per annum (Band 6)
  • Location: Compton, Wolverhampton and working across the community
